The vast majority of men seem to be satisfied with the size and thickness of their penis, however there are those who are really interested in improving their physical appearance and look for ways to achieve it through medical treatments and external elongators. There is also the surgical option called Phalloplasty, which is responsible for achieving the appearance of a virile member longer and thicker.
There is a portion of the penis, located just at its base, which is internally “hidden” at the level of its insertion into the pubic bone. This ligamentous junction is given by the suspensory ligament of the penis and is the key to any phalloplasty technique.
There are several techniques, with different approaches and coadjuvant methods to achieve a better result. Pubic and scrotal approaches have been described, combined with z plastias or rotational flaps, however each technique must be individualized for each particular case. Dr. Camacho-Melo performs his technique with natural spacers using the tissues of the same patient, which help to avoid the retraction process. Do not use spacers or Silicon prosthesis in any Phalloplasty.
The maximum gain that could be achieved in length increase is approximately 5 cm (2 inches), likewise the average gain with thickening of the penile perimeter is similar.
Of the pubic approaches the most requested one is the one of V-Y, where it takes advantage of skin of the pubic area so that it accompanies the penis in its lengthening, leaving it less retained in its base.
We agree with the use of elongators or Stretchers before, during and after the Phalloplasty, as the case may be. It is recommended that after the procedure the elongated limb is kept to avoid retraction of the scar tissue at the base of the penis.
The thickening of the penis can be done with fat grafts from the same patient, with dermograft grafts or alloplastic material such as Belladerm. Dr. Camacho-Melo Does not use PMMA for thickening.
Phalloplasty is contraindicated in patients with Erectile Dysfunction problems.

Thickening can be accomplished by a variety of methods and techniques, including fillers (Hydronical ac), dermal grafts, fat transfer and the use of FDA-approved allopathic materials.
- Duration: Depending on the procedure from 1 to 4 hours.
- Anesthesia: Subarachnoid Blockade.
- Recovery: Sexual abstinence is recommended for 3 weeks.
- Result: The average both length and thickness increases by 1 ½ inches.
